Delaware first, not Delaware only

The deadline is national. The federal Interoperability and Prior Authorization rule (CMS-0057) requires health plans to support electronic, FHIR-based prior authorization by January 1, 2027 — and CMS has been clear that no single organization gets there alone.

Delaware moved first. It wrote shared, neutral infrastructure into its federal Rural Health Transformation Program plan — Initiative 15 — built on vendor-neutral standards and stakeholder governance, so prior authorization gets solved as public infrastructure rather than another point product. Delaware’s published Year-3 targets: 75% faster prior-auth responses, 85% clean claims, 90% fewer coverage-error denials.

The pattern is built to travel. This Connectathon proves it live — the day before the national CMS FHIR Connectathon (July 14–16) — using the same standards every state, payer, and provider is already preparing for. What works in Delaware is designed to replicate state by state. Wherever you are, the work you’re already doing for CMS-0057 makes you ready to join.

What you’ll do

Start with the free CMS-0057 Readiness Check: deploy the Smart Gateway, run synthetic eligibility and prior-auth flows, and get a green / yellow / red conformance report against the real Da Vinci IG profiles — so you can see exactly where your APIs stand before the January 1, 2027 deadline.

   Test it live. The Da Vinci prior-authorization flow plus eligibility checks, end-to-end across the SHN Gateway and Hub — the same testing as the CMS FHIR Connectathon (July 14–16).

   Pass, and you’re ready to route. The same green check that proves you’re conformant is your go-live gate to route production through SHN.
