Delaware first, not Delaware only
The deadline is national. The federal Interoperability and Prior Authorization rule (CMS-0057) requires health plans to support electronic, FHIR-based prior authorization by January 1, 2027 — and CMS has been clear that no single organization gets there alone.
Delaware moved first. It wrote shared, neutral infrastructure into its federal Rural Health Transformation Program plan — Initiative 15 — built on vendor-neutral standards and stakeholder governance, so prior authorization gets solved as public infrastructure rather than another point product. Delaware’s published Year-3 targets: 75% faster prior-auth responses, 85% clean claims, 90% fewer coverage-error denials.
The pattern is built to travel. This Connectathon proves it live — the day before the national CMS FHIR Connectathon (July 14–16) — using the same standards every state, payer, and provider is already preparing for. What works in Delaware is designed to replicate state by state. Wherever you are, the work you’re already doing for CMS-0057 makes you ready to join.
